    First layer advice needed

What do these marks indicate on my first layer? They’re always randomly placed...is my Z height off? When I print a 0.2mm calibration square it measures almost spot on. There are also no bumps or holes in my heat bed to make those impressions. Thoughts?

Watch your extruder temp while printing, it might be dropping down. Also make sure you have a clear filament path and it is not getting caught on anything. Doublecheck your connections to make sure none are loose. You might need a better hot end if you are using a cheap one, I use to have all kinds of print problems until I switched out the hot end with a better one.

It looks like you are having issues with the Extruder not getting enough filament to the nozzle.
I see voids in the print as well as tiny boarders or lines between lines of filament as the nozzle moves diagonally across your print filling in the first layers.
Check that the drive gear on the Extruder stepper motor isn't loose or worn.
You didn't say if you have a Direct Drive or Boden Tube Extruder. Check the Boden Tube connecters and make sure the Boden Tube is pressed tight to the back of the nozzle so there is no filament leakage in the Hot End that could be causing a clog.
Try slowing your print speed down or turning up the Print Temp.

It is a direct drive system - no Bowden.

This is on a Prusa MK2S.

          That information helped a bit.
You should try doing a Google Search for "Prusa MK2S extruder calibration"
I found several photos of first layers like yours posted online, but with various ideas and solutions from the bed height to nozzle replacement to calibrating the extruder and even replacing the filament.

Try this and see if you find a solution for your Prusa MK2S and the filament you're using.
